When you supply your personal details to Radcliffe On Trent Osteopathic Clinic they are stored and processed for 3 reasons:
1) I have a legal obligation to maintain medical notes in order to provide you with treatment. Your medical records include name, address, DOB, medical history health conditions and Medication details, of records of treatment and care provided.
2) I also think it is important that I contact you in order to confirm your appointments with me or to update you on matters related to your medical care. Under the GPR, this is known as legitimate interest.
3) Provided I have consent I may occasionally send you general information in the form of articles. You may withdraw this consent at any time – just let me know.
I am legally obliged to maintain patients records for a period of 8 years from the date of the most recent contact.
In the case of children, I have a legal obligation to retain records until the child reaches 25 years of age
or 8 years from the most recent contact, but after this period you can ask me to delete your records if you wish. Otherwise we will keep your records indefinitely in order that I provide you with the best possible care should you need to see me at some future date.

I will never share your data with anyone who does not need access without your written consent. Only the following people/ agencies will have routine access to your data.
Gp’s consultants, surgeons and /or medical insurance companies
Where this is required I will always inform you first unless I am under legal obligation to comply.
In making initial contact with this practice you consent to me maintaining a marketing dialogue with you until you until you either opt out which you can do at any time by contacting me.
Some basic personal data may be collected about you from marketing forms and surveys you complete, from records of my correspondence and phone calls and details of your visits to my website, including but not limited to, personally identifying information like internet protocol IP address.
